This isn't a bidding poll because it is two parts.  You do not play transfer to spades and bid game as a slam try, it is how you play game by partner.

Perhaps 4S is off on the defence HA, H ruff, club over, H ruff, partner being 3334, say.  Perhaps 3NT is off the club suit.  Perhaps you are making a slam.

Your options are

- transfer to spades and bid 3NT, offering choice of games

- transfer to spades and bid 4S, to play if partner doesn't break the transfer (you can also just bid 4 yourself if you like)

- Transfer to spades and bid 4H, slam try with a singleton heart

- 4, slam try in spades (partner bids 4 very good with a heart control or very bad, 4 medium, higher good without the  A)

partner will not break a 3-level transfer without 4-card support

Offer choice of 4 and 3NT
Play in 4 unless partner breaks the transfer
Show a slam try via 4
Other (explain)
