Two no trump, Jacoby, Game forcing heart raise  Agreements are rather primitive.  first bid shows shortness.  Bids after first bid show controls.

Slow arrival shows extras.  If you have three keys you convert signoff to slam.

West says that East should have bid key card Blackwood after four diamonds. He would have learned that West has zero or three key cards, and  after the  five diamond response,  East  can bid five hearts which West will convert to six.  East knew enough to bid six hearts after five diamonds, and five hearts was a sign off.  By the way, what was that double of three diamonds, we never discussed that.

East says that he showed diamond shortness, controls in the black suits, and made three slam tries, double, three spades, and five clubs, all of which he made instead of signing off at four hearts  West is looking at three key cards and  four controls, including the ace king of hearts.  East has done a lot of bidding missing the ace king of hearts, and West is looking at them so the question should be whether to bid six or seven, not whether to bid six or pass.  Five hearts was a last train in case West didn't have enough to bid slam, but West did have enough and shouldn't pass.  The double of three diamonds was undiscussed, but should be interpreted as showing shortness since the bid for showing shortness was stolen.  BTW, I am missing four key cards, only have thirteen high card points, and had nothing more to show when I bid five hearts.

East West have come to you as their coach/mentor/ arbitrator for counseling.  What  are your comments and advice.

West should have bid six hearts, get over the fact that partner didn't bid key card, you can bid slams without using Blackwood.
East knew enough to either bid six hearts after five diamonds, or should have bid four no trump to see if West had three key cards.
Both are responsible, West more than East
Both are responsible, East more than West
Neither is to blame, five hearts is where this hand belongs.
both bid brilliantly, if they were playing with someone else.
