Matchpoint Pairs. Lead was S7. Declarer won in hand and played Low Diamond from Hand which was ducked by all. E played 8 playing reverse smith (high= asking for shift, low asking for continuation). Then Declarer ran his 4S and 5H tricks to make 10 tricks and -430 for EW which gave EW 3/17. On proper Defence they would score 7 tricks for +150 and 14.5/17

Who's fault majorly?

Mostly E as he should lead a (Mention in comments which one)
Mostly W as he should take first round of Diamond and Switch to Club King after the lead and Smith Echo by E
Both to blame almost equally
