In the Senior Swiss the following deal arose, with both sides vulnerable:

North's sequence did not guarantee a four-card major. There was no alert or other explanation of this before the opening lead was made. West led a club and when dummy came down E-W objected to the failure to alert and called the TD, who instructed that play should proceed.

In the fullness of time the contract went down one and the TD initially ruled that the result stood. 

After consultation with another TD the ruling was changed to down 2, on the grounds that West might have led a heart had he been correctly informed, and that would have led to a two-trick set.
