Poor behavior and Recorder Forms

All the brouhaha that has been created this week regarding poor behavior at the table has generated a lot of discussion regarding the best ways to handle problems at the table. When someone mentions a situation to me I ask two questions. Did you call the director? Did you fill out a recorder form?

While the answer to the first question varies, the answer to the second one is almost a universal NO. The reasons range from a shrug, to a strong opinion that recorder forms do no good.

What are your suggestions for improving the process?

Totally worthless
Causes enemies and does little if any good.
Does some good regarding the problem reported but isn't tracked for future occurances.
Useful in tracking problems with a player or pair.
