Unusual Major Jumps Over One Minor/Diamond Opening (UMJOOMO/(UMJOODO)

The convention Unusual Major Jump Over One-of-a-Minor Opening (UMJOOMO) was presumably invented by Dr. Raghavan and popularized by Meckstroth and Rodwell.  When playing a strong club, it applies only after a 1 opening and thus is called Unusual Major Jumps Over One Diamond Opening (UMJOODO).

1m - (Pass) - 2 = 5 spades and 4+ hearts, constructive but less than invitational values

I have not been able to find any information on the follow-ups to the 2 and 2 bids, such as what 2NT would mean and which bids would be forcing now by the opening bidder.

Does anyone know where such information could be found?  Or perhaps someone could supply some supplementary information on this convention?

A few questions I can think of would be:

1.  Would a bare 5 to 6 HCP hand use this convention?

2.  Is opener's 3m rebid non-forcing and expected to be passed over both the 2 and 2 bids?

3.  What is 2NT over both the 2 and 2 bids?
