I seem to be in the minority with 3, but wouldn't partner have bid 3 at his previous turn if he didn't have heart tolerance?

If partner doesn't want to defend 2XX he can pick a suit or bid 2N to let me pick.

E should rebid 2N. Even with the actual 2 bid, it's unclear whether W should bid 2, 2, 2N, or 3. But anything other than W passing should lead to 3N.

I don't see anything clearly wrong by E or W, just bad luck. South elected to roll the dice and it worked.

E could have opened either 1 or 2, but even without that W has a clear 2 bid showing a good raise.

5 has a small downside if both 5 level contracts are going down and a large upside if either contract is making.

How can pass be forcing? East can easily have a good hand and didn't think 4X would go down enough to compensate for their game.

Agreed. We have no idea how good partner's 2 bid is, 1N the first time gets our values across and leaves our side much better placed.
