Double Dummy Puzzler

I was kibitzing this hand from the Great Britain Bridge Semis (board 29), and the bidding at both tables was the same:

The double dummy lead is a heart to set the contract, but opening leader (South) understandably led a high spade.  And this is where I get confused with the double dummy analysis:

  • To make the contract DD, declarer has to pitch either the spade 7 or spade 6, but not the spade 4, on trick 1!

Given that these cards are all equivalent (declarer has the 5), it seems that it generally is good technique to keep communication with the 5 of spades, but I don't really see which lines it matters in...
