Two competent mid-A players are playing for the first time in a team game.  1NT=15-17.  Limited discussion. Both 2C and 2D are new minor forcing for 1 round,  bidding presumably the better suit.  Having shown a balanced 2=5=3=3 hand (exactly 5 hearts and 2 spades)  South thought 2H would show the minimum(12) and 2NT would show the maximum.  North thought 2NT would show the minimum and 3NT by South would show the maximmm.  The opening leader led a low club from AKxxx. And a game was missed.  Yes we both know 2-way check back, or whatever they call it, could solve this problem, but 15 minutes filling out a new card didn't get into that.  Opinions?

Sorry:  In the text above I mistakenly used South and North for West and East respectively.  The diagram with EW hands and bidding is correct.

South was entirely at fault for missing game.
South was more at fault for missing game
No fault, just unlucky
North was more at fault for missing game
North was entirely at fault for missing game.
