Plan the play in 3NT

I came across this hand (from 1946) recently and thought that it would be interesting to discuss the play.  The bidding supplied is as per the original article and is not recommended (2, 2NT and 3 were all non-forcing).

West leads Q (playing top of sequence) and East plays 2 (standard attitude).

The choices boil down to whether or not to win the first heart and what to do in the diamond suit.

What line do you adopt?

Duck the first heart; if they continue the suit, run 10
Duck the first heart; if they continue the suit, play 10 to the King
Duck the first heart; if they switch to a spade (specify)
Win the first heart and run 10
Win the first heart and play 10 to the King
