There is no history of forgetting Kokish. We play infrequently (1 time per year), and this is the first time Kokish has ever come up. It came up again later in the same event, and we didn't forget it the second time. :)

If he had bid 2, I would have bid 2NT. If he had alerted 2, and then bid 3, I would have thought he had gotten ahead of himself and misbid.

We have never discussed breaking the relay. In describing the bid, we have always used the description "requires a 2 bid".

2 would have been 5+ hearts, 2 of top 3 honors, 8+HCP. 3 would have been 6 hearts, 4-6 HCP, no outside A or K.
